I signed up for Medi-Cal without selecting whether or not Medi-Cal would choose my own health plan and doctor for me. Today, in the mail, I received an application for me to fill out for me to choose for myself my Medi-Cal own health plan and doctor. How do I know that no other accounts have been placed in my own name and/or under any inclusions of my own personal data, for instance, in reference to Medi-Cal choosing my Medi-Cal own health plan and doctor for me?
